kathmandu and manila are probably the dirtiest. i’ve been to india but skipped the major cities except bangalore (it was technically a work trip, but i stuck around for several weeks and did some sightseeing). in africa… cairo. in europe… the turkish republic of northern cyprus, if that counts as europe.

Public restroom off the highway in Morocco. Poo on the walls, on the ceiling, pretty much everywhere but the hole in the ground.
The most dirty place I've visited was a Toi Toi near a huge mountain parking lot. For luck, holding air for a minute is not an issue for me.

I’ve only been to the Americas, Africa, Europe, and Singapore. From my personal experience nothing came close to parts of San Francisco and encampments in the rest of the Bay Area.
FWIW, this is from a few years ago. Tenderloin, Mission, and SOMA are worse now.
