Dr Janis/Lu Kim/various has a multiple personality disorder (which is not the same thing as schizophrenia). Poor Butters just has a very active imagination and basically no friends half of the time.

It really does fuck with me thinking back on episodes. He builds a great wall, runs City Wok and City Airlines, child labor force. How long has he been living as Lu Kim?
I might be alone on this, but I was kinda thrown off by the psychologist’s character design, since most new South Park characters had more realistic oval heads at that point in the series.
This guy had a round head like the character from the older seasons. It made more sense once it was revealed to be the Asian dude.
Biggest plot twist? Finding out all of south park broke the damn.

"Token" as his name never really made sense, if you were to stop and think about it in-universe, but was clearly originally supposed to be an obvious commentary on the "token black guy" in Hollywood, and everyone watching kinda just accepted it and went along with it (further proving the point). His name being "Tolkien" "the whole time" was just retconning and to make the show more fitting to the current cultural climate the show has adopted.

Dr. Janus beating the shit out of Butters is a top 5 comedy moment for me in the show. It is fucking hilarious.
