LIKELY the shingles in itself is the thing leading to strokes and heart attack, because it damages nerves, apparently older and immunocompromised get more atypical symptoms, like it can attack the vagus nerve, or the intestines.
these 2 groups are more likely to get multiple-dermatone shingles, and disseminated too. the one that killed feinstein was encephalitis by shingles, which is more severe than meningitis.
a research paper i came across, where someone had shingles(i believe immunocompromised) had some mild lesions on thier liver.
