Its miscommunication between the HC and special teams coach.. they were probably going to sub a player in and the HC send in Player A and the special teams send in player B..

Someone fucked up and you have 12 men on the field..

Matt Patricia destroyed the Lions, and Josh destroyed the Raiders..
