I've been a bouncer for 10 years, the answer is yes. If someone reeks so bad you can smell it through the cigarettes and general club/event-musk, there has to be more going on.

Do you ever refuse entry for someone who is completely doused in perfume? Or is it only for body odour?

It's never "just" for body odor. Like I said, if there was any way you can smell someone through everything else there's probably more going on with that person. And yes that includes perfume.

In that case, thank you for your service. I, for one, find overly perfumed people to be just as bad as people with bad BO. I've had to cross the street when I was outside because someone walking in front of me was leaving a cloud of perfume stench in her wake.
