It just happened in 2020.
Bill O'Brien led the Texans to the AFCS title in 2019, and beat the Bills in the WC round of the playoffs that year before losing 51-31 to the Chiefs.
After the Texans started the 2020 season 0-4, O'Brien was fired. So not only was he fired the season after making the playoffs, but he was fired 5 games after winning a playoff game.
