KEXP 90.3 Seattle / KEXC 92.7 Alameda-San Francisco
It's a non-profit radio station with eclectic programming and live DJs 24/7 (meaning no automated playlists, ever).

In addition to the previously linked SomaFM and RadioParadise, I use ReListen to listen to the Grateful Dead and a lot of other recorded concerts.

Not solely an internet radio, but i believe they were steaming since day one on the web. No commercials, with djs live on air and music from a wide range of genres. (They do have recurring shows though so you can tune in if there’s something you are interested in) Discovered a ton of great music and artists through them, they really have a great mix.
