Knowing and not acting
Environmental pollution. All the different sorts of environmental pollution.
Knowing well in advance that doom is coming and still not figuring out how to band together and stop it

Probably the destruction of civilization.
Then nothing, as the generations pass and the wreckage of our civilization becomes a simple fact of life rather than a great tragedy.
Killing half of the world's other species on our way out?
Our cowardice in the face of the slow-burn societal and environmental decline. The fact that we had the numbers, but still allowed a very small percentage of rich people to destroy the world around us while we only focused on our individual wants.
Probably greed and ignoring the science behind climate change and ecological collapse

Exploitation of earth's resources and ecosystems with a complete disregard for the future.
