I’ve heard stories from ladies that made my eyes water. Guys they liked and decided to get physical with only to discover they were working with something roughly the size of their own clitoris. I could see there being an issue if you stopped flow of urine in your actual penis but that is controlled by inner sphincter muscles in the bladder not your urethra.
Perhaps his penis is not the only underdeveloped part of his urinary tract. Maybe his bladder sphincters are also underdeveloped and can't hold back a full bladder.

Almost certainly. Gender affirming care also includes things like breast reduction for men, and anything else that is done to make someone appear more like the stereotypical form of a given gender. It doesn't matter if you're assigned that gender at birth either.
